Create Runbooks for operational documentation. For ADRs, tech specs, requirements, LLD and technical analysis use the tech-writing skill instead.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
> **AI-consumed reference.** Optimized for Claude to read during execution.
> Human-readable explanation: see [docs/architecture/HIERARCHICAL_PLANNING.md](../../../docs/architecture/HIERARCHICAL_PLANNING.md)
> or [docs/getting-started/](../../../docs/getting-started/) depending on topic.
# Documentation (ADR & Runbook)
## When to Create
- **Runbook:** Service deployment, common ops tasks, incident response
- **ADR:** → use `skills/tech-writing` + `templates/decision-record.md`. That template carries the
full 4-tier ladder (Y-statement → Nygard ADR → MADR → RFC) and picks the tier by decision size.
The stripped ADR template below is kept only so existing links keep resolving.
## ADR Template
```markdown
# ADR-[N]: [TITLE]
**Status:** Proposed | Accepted | Deprecated | Superseded by ADR-X
**Date:** YYYY-MM-DD
## Context — ## Decision — ## Options Considered — ## Consequences
```
Location: `docs/adr/ADR-NNN-description.md`. Keep immutable — supersede, don't edit.
## Runbook Template
```markdown
# Runbook: [Service]
**Owner:** [Team] | **On-Call:** [Contact]
## Prerequisites — ## Common Operations — ## Troubleshooting — ## Alerts & Escalation
```

The line at the top of this page pins the label git:20261029.30f94d9, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
