## Protocol
1. **Extract** — Scan for correction signals (loops, rejections, shape mismatches, lint rework)
2. **Classify** — Root cause: routing | procedure | example contradiction | workflow | tool/adapter | evaluator | environment; do not patch a skill for a runtime permission failure
3. **Trigger Miss Check** — For every task in session, ask: _" relevant skill available but not loaded?"_
- If yes: record skill ID, indirect phrase used, and fix (add keyword alias to triggers)
4. **Propose** — One evidence-linked candidate per root cause, with status `proposed` until independently reviewed; extend, merge or retire existing guidance before adding skills
5. **Authorize edits** — Without explicit maintenance authorization, return proposal-only. Authorized edits change canonical registry source, not installed copies; regenerate exports through existing tooling
