Audits a codebase and its infrastructure config for HIPAA Security Rule compliance — locates PHI in data models, logs, error trackers, analytics, and outbound LLM/API calls, then reports findings graded against §164.312's technical safeguards with file:line evidence. Use when the user asks to audit an app for HIPAA, wants to know if they're HIPAA compliant, is building or onboarding a healthtech product, mentions PHI, ePHI, covered entity, business associate, Business Associate Agreement…
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# HIPAA Skill Audits a codebase and its infrastructure configuration — not a live system, not a legal filing — against the HIPAA Security Rule (45 CFR §164.312) and reports where PHI is exposed or a required safeguard is unimplemented. HIPAA is unusual among compliance regimes in that most of its failure modes are literally visible in source: a logger call next to a `Patient` model, an unscrubbed error report, a clinical note sent to an LLM with no Business Associate Agreement (BAA) behind it. This skill finds those. It is not a legal opinion, not a substitute for counsel or a risk assessment, and there is no such thing as being "HIPAA certified" — HIPAA is a set of legal obligations enforced by OCR, not a badge to earn. ## Scoping — Do This First Two questions gate everything below. Get both answered — by inspection, or by asking the user directly if the code doesn't make it obvious — before producing findings. **1. Does HIPAA even apply?** Classify the organization's role: | Role | What it means | Applies here when | |---|---|---| | Covered entity | Provides healthcare and transmits health info electronically | The org is a provider, health plan, or clearinghouse | …

How to install
mdr add eagerworks/skills/audit-hipaa@sha256:7acfbf0acef80bb0The line at the top of this page pins the label v1.0.0, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
