Use when reconciling documentation to code changes or auditing documentation drift in a repo. Applies whenever the user asks to update docs after code changes, check whether docs match code, run a documentation audit, or produce doc drift reports; also reviews accumulated work against the repo's mission/intent anchor for alignment; defaults to dry-run proposals and requires explicit apply before writing.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# Document ## Purpose Keep documentation aligned with code without inventing facts or silently rewriting contracts, and keep accumulated work aligned with the repo's stated intent without ever landing an intent-anchor change. Use this skill in two modes: - **code**: reconcile docs ⇄ code truth, from two composable entry points — change-driven reconciliation (default: the working-tree diff) and a corpus drift scan. - **intent**: review a bounded window of the work record against the repo's intent anchor and remediate the findings. Documentation follows code. Do not modify code to match docs, create changelogs, bump versions, author brand-new documentation with no code or work basis, or perform git operations (commit, push, merge, PR) — remediation produces edits, drafts, and ledger items; the user lands them. Never land an intent-anchor edit, under any grant. `intent` mode does not re-check code facts — that is `code` mode's job; intent verdicts cite the work record and doc lines, not code internals. There is no combined code+intent mode and no overall repo health score — a bundle is a call-site invoking both modes, not a mode. No skill eval/benchmark machinery; steering… …

How to install
mdr add moonray/repoprompt-workflows/document@sha256:47d52fab0def53b7The line at the top of this page pins the label git:20260822.91c6099, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
