Builds a well-formed bug (defect) report from raw observation notes - fills in summary, environment, steps to reproduce, expected vs actual, and severity rationale - and validates that each field has the load-bearing content reviewers and engineers need to triage. Also converts a single test-failure record (JUnit XML, Allure JSON, pytest log, Playwright report) into a classified, ready-to-file bug spec, and provides the adversarial review checklist that gates a report before it enters the…
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# bug-report-template ## Overview A well-formed bug report is the input contract for triage, repro, and fix. Industry consensus from Mozilla's long-standing bug-writing guide ([mozilla-bug-writing][moz]) and the ISO/IEC/IEEE 29119-3:2021 incident-report format converges on the same eight fields: [moz]: https://bugzilla.mozilla.org/page.cgi?id=bug-writing.html | Field | Purpose | |-------------------|--------------------------------------------------------------| | **Summary** | Triage line - describes the problem, not the cause. | | **Environment** | OS, build, browser/runtime version, locale, device. | | **Steps to Reproduce** | Numbered, deterministic, copy-pasteable. | | **Expected** | What should have happened. | | **Actual** | What did happen (verbatim error message if any). | | **Severity** | Impact on users (intrinsic to the bug). | | **Priority** | Order of fix relative to other bugs (extrinsic, business). | …

How to install
mdr add testland/qa/bug-report-template@sha256:a3b1f0834cf6973fThe line at the top of this page pins the label git:20260813.bc28491, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
