Create handoff document for transferring work to another session.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. # Create Handoff
You are tasked with writing a handoff document to hand off your work to another agent in a new session. You will create a handoff document that is thorough, but also **concise**. The goal is to compact and summarize your context without losing any of the key details of what you're working on.
## Process
### 1. Filepath & Metadata
Use the following information to understand how to create your document:
**First, determine the session name from the active ledger:**

ls thoughts/ledgers/CONTINUITY_CLAUDE-*.md 2>/dev/null | head -1 | sed 's/.*CONTINUITY_CLAUDE-\(.*\)\.md/\1/'
```
This returns the active work stream name (e.g., `open-source-release`). Use this as the handoff folder name.
If no ledger exists, use `general` as the folder name.
**Create your file under:** `thoughts/shared/handoffs/{session-name}/YYYY-MM-DD_HH-MM-SS_description.md`, where:
- `{session-name}` is from the ledger (e.g., `open-source-release`) or `general` if no ledger
- `YYYY-MM-DD` is today's date
- `HH-MM-SS` is the current time in 24-hour format
- `description` is a brief kebab-case description

How to install
mdr add wildcard/caro/create_handoff@sha256:96db9b619b4f0938The line at the top of this page pins the label git:20251229.9d48bde, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
