Turn a decision the user can't fully answer into a Markdown questionnaire for someone else to fill in, async or over a meeting.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# To Questionnaire Turn something the user can't answer alone into a **questionnaire**: a Markdown document they hand to one person to fill in async, or fill out together over a meeting. The recipient holds knowledge the user lacks; the questionnaire pulls it out of them. **Grill the send, not the subject.** Interview the user only about the _send_, which they can always answer: who it goes to, and what they need back. The questions in the document then target the **gap** between what the recipient knows and what the user needs. Use the question shape from `grilling` for the setup questions, `AskUserQuestion` where available, otherwise chat. The recipient has none of the user's context, so `grilling`'s **Frame the problem, then ask** and **Write it in Simplified Technical English** rules govern the whole document, not just the setup questions. A question the recipient must decode is a question they answer badly or skip. …

How to install
mdr add leek/agent-skills/to-questionnaire@sha256:a340aeedbeadc760The line at the top of this page pins the label git:20261006.cae3ac3, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
