Create and maintain lightweight Architecture Decision Records as agent-readable decision memory: what was decided, why, and what was rejected. Use when "record this decision", "set up ADRs", or "the agent keeps suggesting Y again".
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# docs-adr — Decision memory the next session can load **Degree of freedom: MIXED — T1 is the priority.** What to record `[HIGH freedom]`; format, supersede-not-edit, and present-backfill-first `[LOW freedom — run exactly]`. Install and maintain the repo's decision memory. **Code shows what was decided; nothing shows why, or what was rejected — so every new agent session is free to "improve" its way back to an alternative you already ruled out.** Re-litigated decisions are the slowest form of drift. An ADR makes the decision durable and its reversal deliberate. ## This skill vs neighbors | Skill | Owns | |---|---| | **docs-adr** (this) | Why we chose X; rejected alternatives; INDEX.md | | `plan-docs-sync` | Docs vs *what the code is* — not why | | `handoff` | Session state for the next chat — not permanent decisions | | `docs-writer` | Tutorials / README prose | | `enhance-arch-boundaries` | Mechanical layering; this records the model | | `workflow-housekeep` | Periodic index / status sweep | | `housekeep-backlog` | Parked-work register (`BACKLOG.md`), not decisions | ## How to reason 1. **Observe** — the decision, the rejected alternative, who already tried to reverse it …

How to install
mdr add kensaurus/cursor-kenji/docs-adr@sha256:0566ba31176f229aThe line at the top of this page pins the label git:20260923.68c095e, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
