Verifies what the assistant claims to have done — proves the work against actual state using OpenAI Codex with a read-only tool surface. Use when the user asks to "verify with Codex", "check the assistant's claims", "did Codex actually do what it said", "verify this turn", "prove the work", or wants a trust check distinct from issue review. Different from `/codex-review`, which finds new issues.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
<!-- PORTABLE-CONTRACT:START --> ## Portable contract Capture the prior assistant message verbatim, decompose it into atomic claims, and verify those claims against actual repository state with a read-only Codex consultation where useful. This is claim verification, not issue hunting. Preserve the Report block, five-grade confidence ladder, parser fallback, and explicit unverifiable state. <!-- PORTABLE-CONTRACT:END --> ## Host adapters ### Pi adapter Apply the portable contract above inline and call native `ask-codex` for focused claim checks. Do not claim an isolated verifier context. ### Other hosts adapter Codex, Cursor Agent, Antigravity, Grok Build, Gemini CLI and OpenCode get this skill from their skills folder, not from a plugin. Apply the portable contract above inline and use the Codex call from `../ask-llm-review/transport.md` for focused claim checks: `model: "gpt-6-astra"`, `reasoningEffort: "high"`, `sandbox: "read-only"`. Do not claim an isolated verifier context. <!-- HOST-ADAPTER:CLAUDE-CODE:START --> ### Claude Code adapter …

How to install
mdr add lykhoyda/ask-llm/ask-llm-codex-verify@sha256:797023d15c565a17The line at the top of this page pins the label git:20261002.162238f, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
