## 1. Locate the active transcript
Before fanning out, find this conversation's transcript in Claude Code's per-project transcripts directory at `${CLAUDE_CONFIG_DIR:-$HOME/.claude}/projects/<encoded-cwd>/`. Do not glob across `${CLAUDE_CONFIG_DIR:-$HOME/.claude}/projects/`. That crosses workspace boundaries and reads private chats from unrelated projects.

How to install
mdr add michael-denyer/pstack-claude/reflect@sha256:2e405550bc830649The line at the top of this page pins the label git:20261030.bfaf03e, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
