Scaffold a new project with the standard AGENTS.md hierarchy, CLI tool config, and gitignore.

## Steps
### 0. Run discovery
**0a. Global activation mode** — read this before any other discovery work.
Read `~/.claude/agentic-engineering.json`. Expected shape: `{ "mode": "opt-out" | "opt-in", "profile": "relaxed" | "default" | "strict", "set_at": "<ISO8601>" }`. If missing or unreadable, assume `mode=opt-out` and `profile=default`. When writing this file during init (if creating it for the first time), include `"profile": "default"` as a sensible default.
- **If `mode=opt-in`**: prompt the user before doing any scaffolding:
> "dinostack is installed in opt-in mode. Activate it for this project? [Y/n]"

How to install
mdr add space-dinosaurs/dinostack/ds-init-project@sha256:92cc5d334ef9d76bThe line at the top of this page pins the label git:20261015.14c97b9, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
