Use to triage an incoming production incident report, classify its severity, and route it to the right on-call team. Use when someone reports an outage, degradation, or production error. Do NOT use for routine service requests, feature questions, or non-production environments.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# Incident Triage Runbook ## When to use A production incident is reported and must be classified and routed. Routine requests do not belong here. ## Steps 1. Gather the symptom, blast radius, and first-observed time. 2. Classify severity using `references/severity-matrix.md`. 3. Page the owning on-call team for that service. 4. Open the incident channel and post the initial summary. ## Failure handling - If the owning team cannot be determined, ALWAYS escalate to the incident commander rota. - NEVER close an incident without a documented resolution. ## Example A report arrives: "Checkout is returning 500s for ~30% of users." Classify against the severity matrix (high blast radius → Sev2), page the checkout on-call, and open the incident channel with the summary.

How to install
mdr add dpalfery/kyber-weave/incident-triage@sha256:5b1033bb19165ba0The line at the top of this page pins the label v2.0.1, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
