Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. # Reflect
Mine the current conversation for durable learnings, then route them into skill edits.
Invoke when the user says "reflect" or "/reflect". Skip when the conversation is trivial, off-topic, or already covered by an existing skill the parent followed correctly. One-offs are not learnings.
## 1. Locate the active transcript
Before fanning out, find this conversation's transcript. It is `~/.claude/projects/<slug>/${CLAUDE_SESSION_ID}.jsonl`, where `<slug>` is the session's starting directory with every non-alphanumeric character replaced by `-`. The session id is unique, so this finds it without opening any other chat:

This session's subagent transcripts sit in `<slug>/${CLAUDE_SESSION_ID}/subagents/`. If no path resolves, pass a tight digest of the session instead.
## 2. Spawn three reviewers in parallel
One message, three Agent tool calls, `subagent_type: "pstack:read-only"`. Reviewers need MCP access to look up the tickets, chat threads, and observability traces the transcript references, and `pstack:read-only` keeps it.

How to install
mdr add lucasheriques/pstack/reflect@sha256:16d3833c81b9b7caThe line at the top of this page pins the label git:20261014.8882c71, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
