What the file says
# runtime ## Atomic remembered approvals `api.approve_escalation_with_fact_at` commits a captured approval and one `ReservedFactChange` through the same writer transaction. Both the escalation sequence and prior fact sequence are expectations; either conflict writes neither value and returns `RaceLost`. The helper accepts only a reserved fact key distinct from the decision key. Runtime treats grants and the fact payload as opaque JSON; client policy validation owns their meaning. The existing `approve_escalation_at` remains the one-call door. See protocol 041. `api.edit_reserved_facts` is the same guard without a decision: a list of `ReservedFactEdit` (`ReservedFactSet` of a `ReservedFactChange`, or `ReservedFactRemove` of a cell read at a sequence) applied in one transaction, each under the sequence its caller observed. A cell that moved loses the whole edit as `RaceLost`; nothing is retried, because the caller's read was a question put to a person. Every key must be reserved. The remembered-permissions forget is its one caller (protocol 073). ## Streamed response handoff Generation and poll `RequestSpec` values carry the response entry already …

This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
