为 0—12 岁儿童收集、导入、补充和标准化家庭资料，作为家庭成长 Agent 全年知识底座的证据输入。用于用户第一次搭建家庭成长 Agent，需要填写适龄问卷、通过对话逐步回答、把家长记录/孩子原话/教师反馈/已有文档整理成规划输入，或检查资料是否足以生成全年底座时。先按计划日期和出生日期确定唯一 plan_track，再只读取对应年龄问卷；输出标准 intake JSON、证据地图和缺口清单。不生成成长评估或规划，不做医学心理诊断、发育筛查、学习类型测评，也不读取未经用户明确提供的客户报告文件夹。.

# 儿童成长资料采集 把“家长很了解孩子”转化成可以被家庭成长 Agent 长期读取、核对和更正的证据资料包。这些资料用于建立首次全年知识底座，不只是为了生成一份一次性报告。一次只使用一个年龄轨道。 ## 1. 确定唯一轨道 必须取得计划日期和出生日期，计算完整月龄： - `infant`：0—35 月龄； - `preschool`：36—71 月龄； - `school-age`：72—144 月龄。 不得把 36 或 72 月龄同时归入两个轨道。接近边界时记录当前托育或就学环境，供报告处理过渡，但不改变上述机器边界。超过范围时停止并说明本 Skill 不适用。 ## 2. 选择采集方式 - **表单模式**：按对应问题库生成可填写问卷。 - **对话模式**：一次询问一个主题组，收到回答后再追问最关键缺口。 - **导入模式**：从已有记录中映射字段，并标记来源与未知。 - **审核模式**：只验证现有资料，输出完整度和补充问题。 确定轨道后，只读取一个文件： - `infant` → [references/questionnaire-0-3.md](references/questionnaire-0-3.md) - `preschool` → [references/questionnaire-3-6.md](references/questionnaire-3-6.md) - `school-age` → [references/questionnaire-6-12.md](references/questionnaire-6-12.md) 不得为了“更全面”读取另外两个年龄问题库。 ## 3. 以观察证据提问 优先追问“发生了什么、在什么情境、谁看见、最近一次是什么时候”，不要引导家长选择诊断标签。允许“不知道、没观察过、不适用”。家长和教师说法不同就并列保留。 严格保留每条材料的真实来源。材料由家长汇总不等于每句话都来自家长；遇到“孩子自己说”“教师反馈”等内嵌来源标注时，按实际说话者记录，不能默认归为提交者。无法确定来源就写入 `unknowns`，不得猜测。 `family_goals` 和 `strengths` 必须是带具体情境或证据指向的内容。笼统标签、占位回答和成人对孩子的定性不能用来凑完整度。只要某一必需主题仍有会改变规划方向的关键缺口，就不得把该主题标为已完成。 不要要求真实姓名、具体学校名、家庭住址或联系方式。专业报告只记录其结论的准确转述和来源，不自行解释生长曲线、量表分数或医学数据。完整边界见 [references/safety.md](references/safety.md)。 ## 4. 规范资料 …
