Records architecture decisions as ADRs in the repository's format or MADR and draws C4 diagrams as text. Use when asked for an ADR or architecture diagram, or when a change adds a framework, data store, or integration style, or changes module boundaries.
Collected from public GitHub. Every version is saved exactly as it was and never changes. Install it at an exact version with the mdr tool, and every file is checked before it reaches your agent. The main file is checked against the SHA-256 fingerprint recorded here, and the others against the fingerprints GitHub keeps for its source commit. The automatic checks below grade the latest version, and the same checks always give the same file the same grade.
What the file says
# Recording architecture decisions An architecture decision record (ADR) lets a future reader retrace why the system looks the way it does, which options were weighed, and what the choice costs. Code in this setup carries no comments, so the ADR is where the reasons go. The planning-implementation skill settles decisions with the user, the applying-clean-architecture skill owns layers and dependency direction, the managing-dependencies-and-ci skill vets a dependency before it is added, the writing-documentation skill owns README, CHANGELOG, and other docs, and the writing-commit-messages skill owns commit text. When the repository already has an ADR format, folder, or architecture documentation convention, follow it over this skill. ## When to write an ADR …

How to install
mdr add dayfinggg/openai-codex-agent-skills/recording-architecture-decisions@sha256:b13b91727603cedeThe line at the top of this page pins the label git:20261015.18f69f5, which is what most people want. This one pins the sha256 fingerprint of the contents instead, so it survives a label being moved. Either way the exact version is written to mdr.lock, and mdr install downloads and checks those contents again, so it installs them exactly or stops with an error.
